226 Appendices APPENDIX 6 Moral Craftsmanship Questionnaire (MCSQ) These are all items from the MCSQ 1.0. The development of this questionnaire was explained in Chapter 4 and the MSCQ was used in the empirical study in Chapter 5. This is a translated version of the Dutch questionnaire we used at DCIA; the translation was made with the help of a professional translator. The answer-categories for most items were: a) strongly disagree, b) disagree, c) neutral, d) agree and e) strongly disagree. The frequency- and leadership-items differentiate, with answer-categories a) never, b) several times per year, c) monthly, d) weekly and e) daily. 1. General presence of reflexivity A. Statements 1.
