The open-item 1 on the first page is used in Chapter 2 about the moral dilemmas, and items that are marked with an * are used in Chapter 6 on the outcomes of MCD. This is a translated version of the Dutch questionnaire we used at DCIA; the translation was made with the help of a professional translator.
