168 APPENDICES About the author (Curriculum vitae) Maurice van der Vorst was born on the 10th of June 1973 in Gilze en Rijen, the Netherlands. After graduating from the Theresialyceum (Gymnasium) in Tilburg in 1991, he studied medicine at the Catholic University of Nijmegen (now Radboud Universiteit Nijmegen), where he also attended lectures in theology. From 1996 to 1997, he interrupted his studies to become president of the Nijmegen student association Carolus Magnus. He obtained his medical degree in 2000 and, after an internship in infectious diseases in Buenos Aires, Argentina, became a resident-not-in-training at the internal medicine department of Spaarne Ziekenhuis Haarlem. He started his residency in internal medicine in 2002 at Zaans Medisch Centrum, Zaandam (supervision: dr. R.L.J.F. Loffeld). In 2008, he finished his residency in internal medicine at VU University Medical Center (VUmc; now Amsterdam UMC, location VUmc) in Amsterdam (supervision: prof. dr. S.A. Danner, prof. dr. M.H.H. Kramer), where he obtained his certifications in haematology (supervision: prof. dr. P.C. Huijgens) and medical oncology (supervision: prof.dr. E. Boven) in 2009. In 2010, he received formal training in palliative care and was registered as SCEN (Support and Consultation on Euthanasia in the Netherlands) physician. He became staff member of the division of medical oncology at VUmc (head of department: prof. dr. H.M.W. Verheul) in 2011, and joined the board of the VUmc Expertise Center for Palliative Care. In the same year, he began his PhD trajectory under supervision of prof. dr. H.M.W. Verheul and prof. dr. J. Berkhof. In 2014, he started as medical oncologist and palliative medicine consultant at Rijnstate hospital in Arnhem. Since 2016, he has been a board member of the Dutch Association for Professional Palliative Care (Palliactief). He has been a member of the palliative care committee of the Dutch Society of Medical Oncology (NVMO) since 2018. From 2017 to 2020, Maurice joined the Supportive and Palliative Care Working Group of the Organisation of European Cancer Institutes (OECI). He was member of the Dutch guideline committee on fatigue in cancer patients in the palliative phase (2017-2019), and the Royal Dutch Medical Association (KNMG) guideline committee on palliative sedation (2019-2022). He is co-chairman of the local organizing committee of the 18th World Congress of the European Association for Palliative Care (EAPC), 2023, in Rotterdam. Maurice is married to Hilde Leemreize and they have two children: Ward (2007) and Leonore (2010). They live in Nijmegen.
